Emergency Planning – updates
Patient Lists for Pre-dialysis, HHD, PD (new)
PROMIS 4 > Emergency Planning > Renal Patient List
In the Renal Patient Lists section, filtering patients based on a patient program group can create a group-specific patient list. Renal Patient Lists can be created, edited and saved with PD, Predialysis, HD, and HHD groups of patients for active emergency lists. Patients can be added to an existing active saved patient list of the same modality and included in both kidney care and relevant dialysis lists if they have both statuses open.
- Patients included in an emergency list are identified in red if they have been deactivated, or if the patient’s dialysis type changes and does not match the list’s modality.
- Edit free-text columns on each patient’s line of information either without creating an emergency list or within a saved emergency patient list.
- Kidney Care (Non-Dialysis) Patient List provides option to search for patients with a specified eGFR parameter.
- A column for Interdialytic Weight Gain is available and editable in the Renal Patient Lists.
Home Therapies Map (new)
PROMIS 4 > Emergency Planning > Home Therapies Map
- In the Home Therapies Map, the filter parameters indicate patients with a marker on the map as they are grouped in active emergency lists. Different coloured markers indicate if patients are included or not in the selected active emergency patient list, with an expandable Legend on the map providing a visual explanation.
- Home Therapies Map function provides the ability to add and save selected patients to an existing active emergency patient list. The left pane displays the patients selected on the map.
- Additional patient details are made visible by clicking on a map marker.
Facility Report (updates)
PROMIS 4 > Emergency Planning > Facility Report
- In the Facility Report, the field “Patient Count” has been renamed to “Patient Count (Inpatients incl.)”, and the new label added with calculated value for “Patient Count (Inpatients excl.).”

Hospitalization
PROMIS 4 > Pt Info > Hospitalizations
- The Hospitalization module displays a record of the patient’s hospital visits, which includes information such as event type, visit number, hospital name, and hospital service. New hospitalization event details can be added, and existing hospitalization events can be modified or deleted. Open hospitalization records are easy to identify without an associated discharge record, by a full green circle on the Admissions record under the Event Type column.

- The creation of a PD infection in Dialysis Access links the record to a hospital admissions record.
Hospitalization Related Reports
Renal Program Inpatient Report
PROMIS 4 > Reports > Centre Reports > Renal Program Inpatient Report
- Renal Program Inpatient report displays renal and transplant patients who have been admitted and are currently in hospital. The report can be generated across management centres and has the ability to exclude non-renal transplant patients.
Quarterly Transplant Activity Report
Classic > Transplant > Reports > Patients Lists > Quarterly Transplants (Physician Billing)
- Quarterly Transplant Activity report modified to no longer display the hospitalization Discharge Date.
Admission Registered report
PROMIS 4 > Reports > Centre Reports > Transplant > Recipient > Admission Registered
- The Admission Registered report displays a list of transplant patients currently admitted to hospital. The report provides options to select centre patients and allows output format of either excel or pdf.
Kidney Transplant Referral Form
PROMIS 4 > Patient Overview > Transplant > Referrals > Kidney
The Kidney Referral Form moved from Classic to PROMIS 4. The workflow process remains the same, with improved functionalities.
- The Kidney Transplant Referral Summary is organized into two sections: (A) Absolute Contraindications Assessment and (B) Kidney Referral Summary.
- To complete the Kidney Referral form, 7 sections (tabs) are to be completed in the form.
(A) Absolute Contraindications Assessment
The rules for Absolute Contraindications remain the same, with ability to Add, Edit, or View the record.
- Absolute Contraindications Summary: Absolute Contraindications of a patient can be viewed from the referral form, displaying when it was last updated and the date range of the assessment.
- Absolute Contraindications (Add): Add Absolute Contraindications for a patient who doesn’t have a record in PROMIS.
- Absolute Contraindications (Edit): Absolute Contraindications are modified from the referral form for subsequent referrals.
- Absolute Contraindications description: Updated description of each absolute contraindication.
(B) Kidney Referral Summary
Summary view of Kidney Referral records, with referral number and current referral status.
- From Summary view, able to initiate a new Kidney Referral for a patient who has met absolute contraindications criteria.
- Summary view lists the referrals initiated and referral status of the record. View latest and past referral records.
Kidney Referral Details

Referral Details view provides a summary at the top of the page on the selected referral displaying referring centre, transplant centre, current referral status, status as of date, and status reasons.
- Kidney Referral Details contains seven sections to view and to be updated.
(Tab 1) Screening
View and edit screening information as previously in the Classic Referral form, including general questions, renal biopsy, link referral documents, view prior transplants, antibody profile details, and verify the patient’s primary renal diagnosis.
- Prior blood transfusions and prior pregnancies: the referral can be submitted even if the option selected is “Unknown”. If the patient is male, the Prior Pregnancies field is unavailable.
- View, add, update or delete any previous history of transplants in the Screening form.
- View the latest HLA Antibody profile information; navigates to Recipient Immunology module.
- Ability to view, add, edit or delete linked documents related to renal biopsy, referral notes, or other documents which are uploaded manually or received through interfaces.
(Tab 2) Demographics, Physical Exam
View, update or add information related to patients’ demographics, care providers, blood type and physical examination, with direct navigation to the physical examination and demographics page.
- A patient’s blood type details can only be added or updated if not autoloaded via interface.
- Calculated BMI can be viewed in this tab view.
(Tab 3) Care Providers, Treatment
View and edit Physician and Treatment details as previously in the Classic Referral form, with the ability to update care provider’s details from the referral form and to view a patient’s current renal status, and past renal treatment history.
(Tab 4) Labs
Similar functionality and validity rules as in Classic, however for convenience similar labs have been grouped together where only one test is mandatory to complete. A help icon has been provided for each lab group.
- Group validation of lab results: lab values are valid if the lab visit date is equal to 365 days.
- EBV IGG: the EBV IGG test is mandatory.
- All labs are required within 365 days to submit the referral. Missing and overdue labs (exceeding 365 days) are identified in red.
- If labs are missing or past due, labs will be identified by a checked box against each test. When the PROMIS Lab Req button is generated, the requisition form contains the checked lab tests required to complete the bloodwork.
(Tab 5) Mandatory Tests
Similar functionality and validity rules as in Classic, with improved ease of use. A help icon provides details for several tests. Expanding the test row provides details along with the linked test result document.
- Ability to identify and update mandatory tests missing or overdue.
- Ability to update, preview or download linked documents.
(Tab 6) Additional Test, Consults, Referral
New section provides the ability to add or edit additional consults, referrals or tests needed for processing the referral,. Linked documents can be added.
(Tab 7) Patient Summary Shared
New section provides the ability to add, view or delete a record indicating if the referral information was shared with the GP or patient.
Check Mandatory
The behavior of the Check Mandatory button has similar functionality as in Classic, with exception that a message will pop-up indicating if the referral is incomplete along with the use a triangular warning icon
displayed on tabs that are missing mandatory information.
Kidney Referral Report
When a referral status is updated to “Referral Submitted”, the system verifies if the referral is complete. If the referral is completed, a kidney transplant program is created, and a referral report is generated. The report is saved in Document History under the Tx Referral Form category and provides a copy of the referral information at the time of submission. The report can be printed from the Kidney Referral Details at any time throughout the referral process.
KY Transplant Tracking Sheet
