Post COVID-19 Program Questionnaire
Note: this module is only accessible to Post-COVID-19 Recovery Clinic users. Post COVID-19 Questionnaire assesses health status and informs healthcare delivery for patients affected by COVID-19.

The Summary page displays a history of Questionnaire records with the ability to filter records by Questionnaire progress status, interval, and dates.
The initial Baseline Questionnaire record is completed and subsequent Follow-up Questionnaire records, at the PCR program’s predetermined intervals.
View page displays:
- Progress status (In Progress or Completed),
- Scores for applicable sections,
- Edit record feature (pencil icon).
New Management Centers
Additional PCR Clinics have been added to PROMIS:
- Royal Columbian PCR Clinic
- Abbotsford PCR Clinic
- Surrey PCR clinic (formerly Surrey Memorial PCR clinic) renamed to encompass all patients in Surrey.

- BMI Calculation in Physical Exam > View page: Calculated BMI may be viewed in the patient Physical Exam detailed view.
Diagnostic Reports auto-loaded to Document History
- Diagnostic reports for Post COVID-19 Clinic and Transplant patients auto-load to PROMIS 4 > Document History from interfaces.
- Providers must write “COPY TO: PROMIS” when ordering all diagnostic reporting and lab results in order for reports to be distributed to PROMIS, where an interface exists.

- COVID-19 vaccine brand and dose is now an available immunization option to enter in the PROMIS 4 Patient Infectious Status/Immunization History screen.
