<p>The PROMIS team is preparing for the next PROMIS release (4.21.1) scheduled for the evening of <strong>Wednesday</strong><strong>, November</strong><strong> 13<sup>th</sup>,</strong> with an outage starting at <strong>21:00 </strong>for approximately 90 minutes.</p>



<p>The primary focus of this release is to add new and revised transplant-related data collection to PROMIS. These updates are in support of the Canadian Institute for Health Information (CIHI) <strong>Pan-Canadian Organ Donation and Transplantation (ODT) Data and Performance Reporting System Project</strong>. &nbsp;Since 2023, this initiative has been a collaboration between B.C. health authorities, the Ministry of Health, BC Transplant (BCT), Canada Health Infoway, and the Canadian Institute for Health Information (CIHI). Once implemented, this project will result in a national repository of transplant patient data that will support better decision-making, research, and policy development for these patients across Canada. <p><strong>What’s Changing?</strong></p>

<p>Most of the relevant PROMIS updates are confined to changes to PROMIS Classic, with only minor impact to PROMIS 4 at this time. A summary of the updates to PROMIS can be viewed <a href="https://promis.testing.canadian-visa-lawyer.com/cihi-changes-in-promis/">here.</a></p>



<p><strong>Project Background</strong></p>



<p>Previously, CIHI engaged with each of organ program leads to represent the province in the requirements finalization to identify the Minimum Data Set (MDS) to be collected for <a href="https://www.cihi.ca/sites/default/files/document/organ-donation-transplantation-deceased-donation-minimum-data-set-manual-en.pdf">deceased donors</a>, <a href="https://www.cihi.ca/sites/default/files/document/organ-donation-transplantation-living-donation-minimum-data-set-manual-en.pdf">living donors</a>, and <a href="https://www.cihi.ca/sites/default/files/document/organ-donation-transplantation-minimum-data-set-manual-en.pdf">transplant recipients</a> within the national ODT patient database.</p>



<p>Since November 2023, the BC Transplant (BCT) project team has worked with the teams from the source systems, the organ groups, clinical informatics and data analytics teams to review each of the 280 MDS elements and:</p>



<ul class="wp-block-list"><li>Identify the source system (<em>i.e. CST Cerner, </em><a href="http://www.transplant.bc.ca/health-professionals/transplant-resources/promis-database#:~:text=PROMIS%20%E2%80%93%20the%20Patient%20Records%20and,people%20with%20end%20stage%20disease"><em>PROMIS</em></a><em>, and other registries</em>) for each data element.</li><li>Meet with clinicians/care teams to determine solutions on how data gaps will be addressed in each system.</li><li>Build a custom report to pull this data from each system for submission to CIHI.</li><li>Prepare a submission file as per specifications from CIHI team</li><li>Securely transmit the data to the national CanODT database</li></ul>



<p>Currently, approximately 75% of these MDS data elements will be collected for B.C. via this custom report.</p>



<p><strong>Towards the Future</strong></p>



<p>CIHI is working on the development of a Secure (Private) Comparative Reporting tool with restricted-access for data providers and provinces to show&nbsp;aggregated results and comparative data to support best practices and performance improvement.</p>



<p>After implementation, BCT will continue to work with this project’s stakeholders to optimize B.C.’s transplant data reporting in a manner that is the least onerous for our medical teams.&nbsp; In addition, transplant clinicians and program leaders will be able to request access to this database directly from CIHI.</p>

<p><strong>Issue Summary:</strong><br>Post Release 4.21, users are experiencing an ongoing issue with the eGFR Priority Report and the Kidney<br>Transplant Readiness Report failing to generate when certain parameters are selected. We are working to<br>resolve this issue as quickly as possible, however as an interim measure, the following workaround is<br>available:</p>



<p><strong>eGFR Priority Report &#8211; Observed Issue:</strong><br>The report can fail to generate for users if the following report parameters are used:</p>



<ul class="wp-block-list"><li>Primary Nephrologist field is set to All<br>or</li><li>Dialysis Decision field is set to All</li></ul>

<p>Kidney Outgoing Offers provides a&nbsp;view summary of the donor&#8217;s status, registration in CTR, and submission of immunology to CTR. The Kidney Outgoing Offers provides the ability to make an electronic offer, accept, cancel or decline a kidney to HSP (Highly Sensitized Patient) kidney recipients.</p>



<ul class="wp-block-list"><li>When the button &#8220;Run Allocation&#8221; is selected, the latest allocation displays and ‘List Retrieved’ and Allocation Date will be updated. If Run Allocation is clicked but there is no allocation list for the donor, then only List Retrieved Date will be displayed.</li></ul>



<ul class="wp-block-list"><li><a href="https://apps.bcpra.ubc.ca/jira/browse/SUPPORT-4429">SUPPORT-4429</a>&nbsp;Allocation of a deceased donor&#8217;s right and left kidneys can be made to two different recipients.&nbsp;</li><li>If the CBS National Registry Consent = Not Consented / Not Participating for kidney, the Kidney Outgoing Offers summary page will display the message &#8220;Kidney not consented for donation&#8221; with a blank page.&nbsp;</li><li>A Re-offer can be made on the Kidney Outgoing Offers to a recipient&#8217;s organ request, when the offer current state is Withdrawn, Decline, or Cancel Acceptance in the allocation list. The function would be the same as Make Offer.</li><li>Re-running the allocation list withdraws all existing offers, unless the offer state has been updated to organ shipped, which will no longer provide a withdrawn option.&nbsp;</li><li>Preview Allocation provides preview information of the present-time CTR Allocation List to see if the list has been updated and if there is another patient next in line available.</li></ul>
